Who are Care Managers?
We are dedicated professionals offering a full spectrum of care, experienced in meeting the needs of seniors and disabled individuals. We pool years of experience into our daily practice. Care Managers are Registered Nurses or Licensed Social Workers are most commonly licensed care managers.
We assess for needs, collaboratively develop a full in-depth initial assessment to identify barriers, risks and active problems a client has. In turn we compile our assessment data into a concise report and individualized initial plan of care.
Our plan of care is reviewed, interventions recommended are approved or adjusted to meet the needs of the client and their loved ones.
Once a plan of care is finalized, we deploy all resources (insurance covered, community resources and privately funded) into action, swiftly minimizing the risk of failure – of clients in current situation or setting.
Care managers then monitor the plan put into place, assuring services initiated are delivered.
Care managers advocate and guide throughout all levels of the healthcare maze.
Assess and alter the care plan if needs change or goals are met. This occurs on a rolling basis.
Communicating with those whom the client has consented to have access and care information is another key quarter back role.
